We ask that you arrive approximately 5 to 10 minutes prior to your appointment time to ensure we can serve you in a timely manner.
Upon arrival for you will be provided with a Client Intake Form.
We ask you to take a moment to carefully complete the form, identifying any areas of pain, discomfort or concern that you would like addressed during your treatment. Please carefully read through any hand-outs you are given as additional written instructions, specific to your treatment may be provided to you for review.
We request that you turn off your mobile devices, upon entering our office, so as not to disrupt the environment of relaxation being created for our clients.
